Care Connections has highly skilled professional Licensed Clinical Social Workers that are State board licensed and approved by the National Social Work Examiners Office for Nevada, Arizona, Pennsylvania, Massachusetts,  New Jersey and New York.  Care Connections therapists are able to diagnose, treat and assist in an array of areas of Mental and Behavioral Health, ranging from Mild to Severe Conditions.  Care Connections is specialized in Crisis/Trauma Intervention Therapy Care connections’ Motto and goal is  “Healing from the Inside Out with Caring Providers Connecting You to Quality Services” through the guidance of a licensed Therapist.

Dr. Ebony Brushbreaker, LCSW

Ebony Brushbreaker is a Licensed Clinical Social Worker, graduating Summa Cum Laude from Colorado State University with a Bachelor’s in Social Work. Prior to her social services career, she served in the United States Army for eight years, receiving an Honorable Discharge as a Staff Sergeant stationed at Fort Bragg, North Carolina. With a Master’s in Social Work, she ventured into the Medical Social Work field, serving for over 12 years in settings including Long Term Care, Hospice, and Home Health. She also held the role of Regional Director of Social Services in Long Term Care.

As an enrolled member of the Sicangu (Rosebud) Lakota Sioux Tribe of South Dakota, Ebony advocates strongly for mental health services within the Native American/Indigenous population. Recently awarded a PhD in Health Administration in Health Care Leadership with honors (Summa Cum Laude), she remains committed to increasing access to medically necessary services. Her cultural background deeply influences her clinical approach, emphasizing holistic and culturally informed treatment, particularly sensitive to the BIPOC experience. Ebony’s eclectic approach to mental health incorporates elements from cognitive behavioral therapy (CBT), expressive arts, structural, and solution-focused therapy.

Ebony works with individuals (age 13+) and families facing a broad spectrum of clinical concerns, including depression, anxiety, substance abuse, history of sexual trauma, grief, adjustment to aging, chronic illness, and end-of-life issues. Outside of work, Ebony is actively involved in her community as a Jingle Dress Dancer, participating in various styles and ceremonies since the age of two.
